Addicted Gamblers Banning Themselves from Arizona Casinos

ARIZONA -- As reported by News 13: ``People who have a problem with gambling can actually ban themselves from all Arizona casinos. It's called a self-exclusion form and it's available on-line to anybody who has a problem.

``The form must be filled out in its entirety and signed by the person who's voluntarily seeking self-exclusion. The state will then send the information, including the person's photo to all tribal gaming offices. It's the gambler's choice whether to be banned for one, five or 10 years from Arizona casinos.
